Transition Services

Beginning at age 14 and continued every year, a statement for transition service needs is included in the student’s Individualized Education Program (IEP). This statement creates a general vision for the student’s future, focusing on required academic classes, related services (i.e. speech, occupational, physical and/or psychological therapies), planned community experiences, vocational trainings and any additional identified supports that will benefit the student’s learning. As part of The Pre-vocational Training Program, The Arc Mercer provides a professional transition coordinator/job coach who works with the team comprised of the student, family members, teachers, staff and other professionals to develop a meaningful transition plan that addresses the student’s individual needs.

Beginning at age 16, the statement of transition services will provide a specific blueprint of academic and vocational services to be provided by the school and outside agencies to ensure the student is prepared to achieve the established goals upon graduation. All goals are modified as the student grows and makes informed choices about their future.

For a transition plan to be successful, best practices recommend that a Division of Vocational Rehabilitation (DVR) counselor and /or Division of Developmental Disabilities (DDD) representative be involved with at a minimum, the student’s annual IEP meeting. The transition coordinator initiates contact with state agencies as appropriate and ensures the student and their families are informed of adult services available upon graduation.

The Arc Mercer will link students with post-graduate services as needed, including:

  • Support for acquisition of competitive employment (DVR based)
  • Continued community based job coaching services (DVR/DDD based as appropriate)
  • Assistance with enrollment in certification program of their choice
  • Assistance with enrollment in continuing education (at school of their choice)
  • Additional transition planning (if needed)
