Individual Habilitation Plan
An Individual Habilitation Plan (IHP) is a written plan of intervention and action, outlining both the prioritized goals and objectives being pursued by each individual. The IHP is a single plan that includes all aspects of the individual’s life, such as relationships, residential, educational, behavioral, recreational, vocational, and medical. A team of those involved with the individual meet once per year to discuss how to better serve and assist them in achieving their dreams.
The IHP is designed to empower and improve the individuals' decision-making abilities through goal development/attainment. Each goal outlined in the IHP is person centered; focusing on the individual strengths, as well as areas needed for improvement both in efforts of preparation for their future. The IHP goals and objectives should assist the individual in the following areas of life:
- Facilitate connections and relationships
- Maximize independence
- Enhance self worth
- Encourage self determination
- Enhance physical and emotional well being
